The following is the online registration form for SMA's CPR Course.

2) Course fee (inclusive of 7% GST) is payable upon registration:-

S$150.00 (Non-SMA/SNA member)
S$135.00 (SMA/SNA/SHF member)

3) Please be informed that our policy on the quantum of refund for the courses is as follows:-
- Absent without notification (no show): no refund.
- Less than 2 weeks' notice: 25% refund.
- Less than 3 weeks' notice: 50% refund.

4) Please choose from the course dates below. We will try to match the course dates requested. However, this is subject to space availability. We will notify you one to two weeks before your course date.
