Procedure For Submission of A Complaint

Thank you for your enquiry.

Please be informed that this is our procedure for submission of a complaint:

1) Please forward an original, signed letter, giving full details of the matter, to

2) In the letter, please furnish the patient's particulars: NRIC No./ Address/ Telephone Number.

3) The doctor's particulars: Name / Place of Practice (Clinic / Department & Hospital Name).

4) A letter of consent from the patient for the release of all relevant information pertaining to consultation and treatment with the doctor(s) or hospital mentioned in the letter. Please download the appropriate consent/authorisation form.

a) For standard complaint
b) For complaint involving minor
c) For complaint relating to deceased patient

5) If you are not the patient, but are submitting the letter on behalf of the patient, kindly furnish your Name, NRIC No., and Contact Address.

Our preliminary enquiry procedure entails obtaining comments from the doctor(s) referred to in the case. Please download a copy of Information on SMA Complaints Committee for your information.

The Committee meets once a month to consider cases for which the preliminary enquiry is completed. While we endeavour to expedite the processing of letters received, the whole process may take 2 to 6 months before you will receive a final reply from us.
